Please use a separate order form for each portrait.  Print and send to the address below along with the photo or photos and a 25% deposit.  Make checks payable to Betty Vandenack.  The balance is due on completion of the portrait.  The artist retains all rights to artwork created.

          Professional photos must be accompanied by a copy right release from the photographer or studio.
